Whenever you take damage, you gain 2% of that unmitigated damage as Attack Power. This Attack Power then decays over time, losing 5% of its value every second unless you dodge a melee attack. Vengeance also no longer has a hard cap.
Instead the decay rate will even out relative to the incoming damage to give you a somewhat constant level of Vengeance. This will take a little bit to occur, however a "floor" value based on incoming damage still exists. So once you take damage you should get an initial burst of Vengeance, and then ramp up from there.

The one problem that has not currently been solved is tank swaps. For now do your co-tank a favour and stop attacking for 3-5 seconds during tank swaps. Once you are at around 95% threat, you can start attacking again. This will at least give them a fighting chance of holding aggro after they taunt off of you and your massive Vengeance stack.
Rage
Rage is no longer generated from taking damage. Instead Guardians now generate Rage from landing attacks on our enemies. Once 5.0.4 hits, Rage will come from two different kinds of actions.
